The Team Kentucky Office of Medical Cannabis Patient and Caregiver Registry Portal is open for applications for medical cannabis cards.
TEAM KENTUCKY OFFICE OF MEDICAL CANNABISIn-State Qualified patient: Adults with a qualifying medical condition who have obtained a written certification from a registered medical cannabis practitioner.
Visiting Qualified Patient: Individuals with out-of-state registry ID cards and documentation of a qualifying medical condition.
in-state minor qualified patient: Individuals who are less than 18 years old with a qualifying medical condition who have received custodial parent or legal guardian consent and a written certification.
Designated Caregiver: Individuals who assist qualified patients with purchasing and/or administering medical cannabis.
TEAM KENTUCKY OMC: QUICK REFERENCE GUIDESIn order for an individual to be eligible for a qualified patient registry ID card in Kentucky, they must be diagnosed with one (1) of the following qualifying medical conditions: Any type or form of cancer; Chronic or severe pain; Epilepsy or other intractable seizure disorder; Multiple sclerosis, muscle spasms, or spasticity; Chronic nausea or cyclical vomiting syndrome; or Post-traumatic stress disorder.
Team Kentucky OMC: REGISTRATION GUIDEThe Program has also issued a Qualified Patient & Designated Caregiver Guide. The Guide walks applicants through the steps to register, including the required info and documents they will need to complete the card application process.
